Request to send “Away from Home Care Package” to College Students or member of the Military who are living away from home.

If in College, what is the Recipient’s year or class level. 1st/Freshman, Grad Student, etc.
Recipient's Address(Required)
Use Address Line 2 to be specific with the address such as building number/names, etc.
Recipient's home Parish(Required)
Input if there are any special considerations for this specific recipient.
Requestor's Name(Required)
Person requesting the package to be sent

Due by Monday, October 30

Please have names, addresses, etc. and any donations to the Blessed Sacrament Parish office by the due date.
